　ES1 FR の水没リークで、両手押しS/Wで起動させ、ワークをクランプしている間に							
　ワークについているスパッタを取り払おうとして、うっかり手を出してしまい							
　左手の薬指の先端を、ワークとクランプの間に挟み骨折							
							
　『動く設備には手を出さない』という事例							
							
　１）リーク上昇中に、既にワークを手で掴んでいる							
　２）自動アンクランプ中に、既にワークを手で掴んでいる							
							
　これらを無意識にやっている可能性もありますので、注意してください。							
							
　もし、ルールが守れない状況がある場合は、ご相談お願いします。							
							
　それと、現在同じような設備に対し、順次対応していますので							
　クランプし切るまで長押しをお願いいたします。							
							
							
２・ルール違反について							
							
　確信的なルール違反をしている方がいます。							
　・報告なしに離席している							
　・スパッターカバーをしないで手直しをする							
　							
　こういった行動は、うっかりやってしまうような、人間本来のミスとは違い							
　自分でコントロールできる領域です。							
							
　確信的な違反行為をして問題が起きた場合							
　殆どの責任は、本人になりますので、絶対にしないようにお願いします。

　ES1 FR的水没检漏作业中，通过双手按下S/W启动，在夹紧工件的同时
　为了清除工件上的飞溅物，不经意间把手伸了进去
　将左手无名指尖夹在工件与夹具之间导致骨折
　这是“不要把手伸向运动中的设备”的案例

1）检漏台上升时，已经用手抓住了工件
2）自动松夹时，已经用手抓住了工件

这些动作可能是无意识中做出的，请大家注意。

如果遇到无法遵守规定的情况，请与我们商量（汇报）。

另外，目前针对类似的设备正在依次进行对应，
请一直按住直到完全夹紧为止。

2・关于违反规则
有人存在明知故犯的违规行为。
・未报告擅自离席
・不使用飞溅防护罩进行返修作业

这类行为与因粗心大意而导致的、人类天生的小失误不同，
而是自己能够控制的范畴。

如果因明知故犯的违规行为而引发问题，
大部分责任在于本人，请绝对不要这样做。

October Morning Assembly Communication Matters

1. September 8th ... Industrial Accident Occurred
   During the submersion leak test of ES1 FR, started by pressing dual-hand switches, while clamping the workpiece,
   trying to remove spatter on the workpiece, inadvertently reaching in,
   getting the tip of the left ring finger caught between the workpiece and the clamp, resulting in a fracture
   This is a case of "Do not touch moving equipment"

1. Grabbing the workpiece by hand while the leak test table is rising
2. Grabbing the workpiece by hand during automatic unclamping

These actions may be performed unconsciously, so please be careful.

If there are situations where rules cannot be followed, please consult us.

Also, as we are sequentially upgrading similar equipment,
please press and hold until fully clamped.

2. Regarding Rule Violations
   Some people are intentionally violating rules.
   ・Leaving the workstation without reporting
   ・Performing rework without a spatter cover

Such actions are different from inherent human errors like careless mistakes,
and are within one's own control.

If a problem occurs due to an intentional violation,
most of the responsibility lies with the individual, so please make sure never to do it.

部品詰め替えに関する事例と注意

発生ライン：924EPF
発生日：R8年9月4日
発生内容：パイプ単品がポリ容器内で2品番混載
発生状況：
ライン内でパイプを治具にセットしようとしたらセットできず、違うパイプが投入されていることに気付き監督者へ報告。
パイプのポリ容器の中を確認したら2品番混載していた。
ポリ容器のパイプAC740 12本（端数）の上にAC700が7本（端数）
⇒現品票はAC740 12本で、AC700 7本の現品票は無い

調査内容：
発見当日
・（EPF1課）夜勤該当作業者へヒヤリング
・（PIPE課）該当品番のパイプの混入がないか選別（968本選別）、生産実績確認　
翌日
・（生産管理部）BHTの在庫移動履歴確認、物流G作業者へヒヤリング
・（EPF1課）昼勤該当作業者、夜勤該当作業者へヒヤリング

調査結果：
9月4日　15：00頃、N1号棟部品ストア→ラインへEPF1課が所有するBHTでAC740 12本、AC700 7本を在庫移動している履歴あり。
その為、部品ストアの時点ではそれぞれ端数品で現品票が付いている状態で在庫が存在していた。
⇒EPF1課の誰かが部品を詰め替えたことになるが該当者が誰もいません。

お願い：
①部品の詰め替えはやらないでください
混載してしまった場合誤組み付けしてしまう可能性があります。
見分けがつかない部品が多数あるので、一律して全部品詰め替えは禁止としています。

②正直に申し出てください
該当者がいれば、なぜやってしまったのかを知りたいだけです。
作業性に問題があれば改善の検討したり、次のアクションにつなげられます。

今回、特定するための調査に複数名の人が時間をかけて調査しています。
生産実績の確認、単品選別、BHT移動履歴確認、まとめ→上司へ報告→展開
EPF1課≒3時間、PIPE課≒3時間、生産管理部≒1時間
計6時間　無駄な時間が発生しています。

5月8日にも同様の事例があり、
925Rラインで組付けようと圧入cpを手に取ったらキャタが割れていた。
落下させた形跡があり、4時間かけて調査したが、該当者がいませんでした。

上記の事例を踏まえ、今一度考えを改めて、起きたことの事象を正直に報告して下さい。

零件重新包装案例研究及注意事项

受影响生产线：924EPF

发生日期：2026年9月4日

事件详情：两个不同零件编号的管材混装在一个管材容器中。

情况：

工人在尝试将一根管材安装到生产线上的夹具中时，发现无法安装。工人注意到插入的管材与原先的不符，并向主管报告。

检查管材容器内的物品后，发现两个不同零件编号的管材混装在一起。

一个聚乙烯容器内装有12根AC740管材（数量不符），容器上方还装着7根AC700管材（数量不符）。

⇒ 产品标签显示的是12根AC740管材，但没有7根AC700管材的产品标签。

调查详情：

发现当日：

• （EPF1部门）对涉事夜班工人进行访谈。

• （管道部门）对混入的同型号管道进行分类（已分类968根管道），并核对生产记录。

次日：

• （生产管理部门）核实BHT库存移动记录，并与物流组工作人员面谈。

• （EPF1部门）与相关白班和夜班工作人员面谈。

调查结果：

9月4日15:00左右，有记录显示EPF1部门使用其所属的BHT将12根AC740管道和7根AC700管道从N1楼零件仓库运送至生产线。

因此，零件仓库中存在贴有产品标签的库存，但数量均为奇数。

⇒这意味着EPF1部门有人调换了零件，但尚未确定具体人员。

要求：

① 请勿调换零件。

零件混装可能导致装配错误。

由于许多零件难以区分，我们全面禁止互换任何零件。

② 请坦诚相告。

如果有人对此负有责任，我们只想知道他们这样做的原因。

如果工作效率存在问题，我们可以考虑改进，并以此指导未来的行动。

这次，多人正在调查以找出肇事者。

检查生产记录、整理单个零件、检查BHT移动记录、汇总→向主管汇报→部署

EPF1部门约3小时，PIPE部门约3小时，生产管理部门约1小时

共浪费6小时。

5月8日也发生过类似事件，

在925R生产线上，一个压入式CP被取出进行组装，发现其轨道断裂。

有迹象表明它曾被掉落，经过四个小时的调查，仍未找到肇事者。

根据上述案例，请重新考虑您的处理方式，并如实汇报所发生的情况。

Case Study and Caution Regarding Part Repackaging

Line: 924EPF
Date of Incident: September 4 (Reiwa
Incident Details: Two different part numbers were mixed together in a single plastic container.
Situation:
An operator attempted to load a pipe into a jig on the line but could not; realizing a different pipe had been loaded, they reported the issue to their supervisor.
Upon checking the plastic container, it was found to contain a mix of two part numbers:
7 units of AC700 (remainder stock) were placed on top of 12 units of AC740 (remainder stock).
⇒ The inventory tag indicated 12 units of AC740; there was no tag for the 7 units of AC700.

Investigation Details:
Day of discovery
・(EPF1 Section) Interviewed the relevant night-shift operator.
・(PIPE Section) Sorted parts to check for further contamination (968 units checked) and verified production records.
Following day
・(Production Control Dept.) Checked BHT inventory transfer history and interviewed Logistics Group operators.
・(EPF1 Section) Interviewed the relevant day-shift and night-shift operators.
Investigation Results:
Records show that around 15:00 on September 4, an inventory transfer of 12 units of AC740 and 7 units of AC700 was performed using an EPF1 Section BHT from the Building N1 Parts Store to the line.
This indicates that at the Parts Store stage, the items existed as separate "remainder" lots, each with its own inventory tag.
⇒ This implies that someone in the EPF1 Section repackaged the parts, yet no one has come forward.

Requests:
① Do not repackage parts.
Mixing different parts can lead to incorrect assembly.
As many parts are difficult to distinguish from one another, repackaging of any kind is strictly prohibited.

② Please report it honestly.
If the person responsible comes forward, we simply want to understand why it happened.
If there are issues with workability, we can consider improvements or determine the next course of action.
This incident required multiple people to spend significant time investigating to identify the cause. The process involves checking production records, inspecting individual units, verifying BHT transfer history, and compiling the information—followed by reporting to a supervisor and disseminating the findings.
This takes approximately 3 hours for the EPF1 Section, 3 hours for the PIPE Section, and 1 hour for the Production Control Department.
That totals 6 hours of wasted time.

A similar incident occurred on May 8th:
When a press-fit component was picked up for assembly on the 925R line, the carrier was found to be cracked.
There were signs that it had been dropped; although an investigation was conducted over the course of four hours, the person responsible could not be identified.

In light of these incidents, I ask you to reconsider your approach and report the facts of what occurred honestly.

本日、CG3Sで訓練中の作業者について
班長より「作業着の背中が溶けているようだ」と報告がありました。

確認したところ、1名作業で1工程のみを訓練していましたが
手待ち時間が長かったため、工程間シュートを背もたれ代わりにして寄りかかっていたとのことでした。

工程間シュートは、部品を次工程へ流すための設備です。
人が寄りかかることを想定して設計されたものではありません。
もし背もたれとして使用することを前提としていたのであれば
それに応じた強度や保護カバーなどの安全対策がされているはずです。

また、昨日はラインサイドの部品シュート本体を
台車の代わりとして部品置き場まで手押しで移動させている事例も確認されました。
部品シュートは運搬用ではないため、重心や転倒防止などの対策は考慮されておらず
転倒災害につながる危険な状態でした。

このように、設備や治具を本来の用途以外で使用すると
設計時に想定されていない事故につながる可能性があります。

設備・治具・台車などは、必ず本来の目的に沿って使用してください。

【監督者へ】
以前もお伝えしましたが、「1名で1工程だけだから安全」という考え方はしないようお願いします。

1名1工程での作業は、通常とは異なる作業形態であり、立派な変化点です。

例えば次のようなリスクが発生します。
・待ち時間に妄想で現実逃避する（集中力低下）
・教わっていない、できそうな作業を行う
・溶接光を長時間浴び続ける
要は、手待ち時間に通常とは異なる行動を取るということです。

また、実習生にシャドウを実施させる場合も同様です。
「付いているだけ」「見ているだけ」と考えがちですが
作業者同士が振り向いた際に接触・転倒するなど、通常とは異なるリスクが存在します。

「1名だから」「見学だけだから」と判断せず
立派な変化点で、リスクが増えているという認識を持ち
普段以上に現場を観察するようお願いします。

今天，在CG3S接受培训的作业人员发生了以下情况。
小林H发现并报告：“这名作业人员工作服背部似乎被烧坏了。”
经确认，该作业人员当时独自负责1个工序。
由于等待时间较长，他将工序间滑槽（Chute）当作靠背，身体一直靠在上面。
工序间滑槽的用途是将零件输送到下一工序，并不是供人员倚靠而设计的。
如果设备本来就允许人员倚靠，那么在设计时就应考虑相应的强度、防护罩等安全措施。
另外，昨天还发现一起案例：
有人将生产线旁的零件滑槽本体当作台车使用，手推着移动到零件存放区。
由于该设备并非运输工具，因此没有考虑重心稳定性、防倾倒等设计，存在发生倾倒事故的危险。
像这样，将设备、治具等用于设计用途以外的目的，会产生设计阶段未曾考虑的危险，可能导致意想不到的事故。
设备、治具、台车等，请务必按照其原本用途使用。

【致各位监督者】
之前也反复强调过，请不要认为：
"只有1个人、只负责1个工序，所以很安全。"
1人负责1个工序，本身就是一种变更点（变化点）。
发生变更点时，可能出现以下风险：
• 等待时间过长，注意力下降，思想开小差；
• 操作平时并不负责的工序；
• 长时间暴露在焊接弧光下；
• 总之，在等待时间内采取了平时不会发生的行为。
实习生进行Shadow（跟班学习）时也是一样。
不要认为只是"跟着看"、"只是观察"就没有风险。
例如，作业人员转身时相互碰撞、跌倒等，都属于平时不会发生的风险。
请不要因为"只有1个人"或"只是见习"而掉以轻心。
一旦发生变更点，就意味着新的风险已经增加。
请比平时更加认真地观察现场，并做好指导。

Today, the following case was found involving a trainee working on CG3S.
Kobayashi H reported that the back of the operator's work clothing appeared to have been burned.
An investigation found that the operator was working alone on a single process.
Because there was a long waiting time, he had been leaning against the inter-process chute as if it were a backrest.
The inter-process chute is designed to transfer parts to the next process.
It is not designed for people to lean against.
If it were intended to be used as a backrest, it would have been designed with appropriate strength, protective covers, and other safety measures.
Yesterday, another similar case was also observed.
A worker used the parts chute itself as a cart and manually pushed it to the parts storage area.
Since the chute is not designed for transportation, its center of gravity, stability, and tip-over prevention were never considered during design, creating a serious risk of overturning.
Using equipment or fixtures for purposes other than their intended use creates hazards that were never considered in the original design and may result in unexpected accidents.
Always use equipment, fixtures, carts, and other devices only for their intended purpose.

To All Supervisors
As mentioned previously, please do not assume:
"It's safe because only one person is working on one process."
One operator assigned to one process is itself a change point that differs from normal operations.
Typical risks at change points include:
• Daydreaming or losing concentration during waiting time;
• Performing work that is not normally assigned;
• Prolonged exposure to welding arc light;
• In short, engaging in behaviors that would not normally occur during standard operations.
The same applies when trainees are performing shadow training.
Do not assume that "they are only watching" or "they are just following someone."
There is still a possibility of unusual risks, such as workers colliding with each other or falling when turning around.
Please do not underestimate these situations simply because "there is only one operator" or "they are only observing."
Whenever a change point occurs, new risks are introduced.
Please observe the workplace more carefully than usual and provide appropriate supervision and guidance.

２．休日出勤される皆さんに
　①労働災害防止
　　◎連休中の出勤は普段と違う作業が多く、潜在危険が増えます。 また、連休中という事
　　　で気の緩みもあり、安全への注意力が欠けがちになります。 作業を始める前には、
　　　安全面を十分配慮した作業手順を決めて、作業に着手すること。
　　◎設備工事や点検整備作業による火災の危険も多くなります。 ガス切断機や溶接機を
　　　使用する際には周辺に可燃物がないことを確認すること。　
　　　特に、シンナー・塗料類や排気ダクト周辺には十分注意すること。
　　◎二人以上の組み作業を行う場合には、安全確認の合図を作業前に確認し徹底すること。
　　◎保護具の着用を必ず守ること。
　　◎1人作業は極力避け、メンバーの安全（KY）を確認しつつ、作業に着手すること。
３．工事関係を管理監督する皆様に
　　◎工事業者の責任者に作業開始前の作業確認と安全の注意事項指示を徹底させましょう。
　　◎関係部署以外（危険が判らない場所）には立ち入らないよう指導しましょう。
　　◎作業終了後の後片付け、清掃確認および火気使用後2H点検を徹底させましょう。

二、致假期期间上班的员工
① 防止工伤事故
假期期间的工作内容往往与平时不同，潜在危险增加，同时容易因假期而放松警惕。开始作业前，请充分确认安全风险，制定安全作业步骤后再开始工作。
设备施工及点检维修期间，火灾风险较高。使用气割设备或焊接设备前，请确认周围没有易燃物。
尤其要注意稀释剂、油漆类以及排气管道（排风管）**周边的火灾危险。
两人及以上共同作业时，请在作业前确认并统一安全联络信号。
必须正确佩戴个人防护用品（PPE）。
尽量避免单人作业，在确认同伴安全（KY危险预知）后再开始作业。
三、致负责施工管理和监督的管理人员
督促施工单位负责人在开工前做好作业确认，并向作业人员充分说明安全注意事项。
指导施工人员不得进入非相关部门及不了解危险情况的区域。
作业结束后，必须确认现场整理、清扫完成，并严格执行使用明火后2小时火灾巡检。
请各位管理、监督人员：

Important Reminders for the Summer Holiday Period

The company will be closed for the summer holidays from August 8 to August 16. Please carefully observe the following safety precautions.

   Holiday work often involves tasks that differ from normal operations, increasing potential hazards. People also tend to become less cautious during holidays. Before starting work, carefully assess safety risks, establish safe work procedures, and then begin the task.
   Maintenance work and construction activities increase the risk of fire. Before using gas-cutting equipment or welding machines, make sure there are no combustible materials nearby.
   Pay particular attention to areas containing paint thinners, paints, and exhaust ducts.
   When working in teams of two or more, confirm communication signals and safety procedures before starting work.
   Always wear the required personal protective equipment (PPE).
   Avoid working alone whenever possible. Confirm the safety of all team members (KY: Hazard Prediction) before beginning work.
3. For Managers and Supervisors Responsible for Construction Work
   Ensure that contractors' supervisors conduct pre-work safety checks and thoroughly instruct workers on all safety precautions before work begins.
   Instruct contractors not to enter areas outside the relevant work zone or any area where hazards are not fully understood.
   After work is completed, ensure that cleanup and housekeeping are finished and that a 2-hour fire watch after any hot work is carried out.

１・実習生が新しく入職しましたが、溶接の光が顔に当たっている位置にいました。
　１サイクルでは一瞬かもしれませんが、何往復もすると『目の火傷』をします。
　新人が入職した際は、必ず『溶接の光の当たり方』を注意してください。
　監督者も、『見学だけだから怪我しないだろう』という安易な考えは止めて
　光の当たり方に問題はないか、現場を確認して下さい。

１. 一名新员工开始工作时，其站位导致焊接产生的强光直射面部。
尽管单次作业周期内的暴露时间可能很短，但反复暴露仍可能导致“眼部灼伤”。
每当有新员工入职时，务必向其告知直视焊接强光的危害。
主管人员切勿抱有“只是看看而已，不会受伤”的侥幸心理；
请务必检查作业现场，确保不存在光线暴露方面的隐患。

1. A new trainee started work but was positioned where the welding light was shining directly onto their face.
   While the exposure might last only a moment during a single cycle, repeated exposure can lead to "eye burns."
   Whenever a new employee joins, be sure to warn them about the dangers of direct exposure to welding light.
   Supervisors must also avoid the complacent assumption that "they won't get hurt just by watching";
   please check the work site to ensure there are no issues regarding light exposure.

２・他の課で起きた軽微な労働災害の報告です。
激しい動きは、災害時の怪我の度合いが大きくなります。
特にライン外作業をする場合は、いつもと違う場面に遭遇する確率が高くなりますので、出来る限りゆっくりと、慎重に作業をするようにお願いします。

2. 本报告涉及在其他部门发生的一起轻微工伤事故。
   在发生事故时，剧烈或突然的动作可能会导致更严重的伤害。
   特别是在执行标准生产线以外的任务时，遇到不熟悉情况的可能性较高；因此，请务必尽可能缓慢、谨慎地进行操作。

２. This is a report regarding a minor workplace accident that occurred in another department.
Violent or sudden movements can result in more severe injuries during an accident.
In particular, when performing tasks outside the standard production line, there is a higher likelihood of encountering unfamiliar situations; therefore, please carry out your work as slowly and carefully as possible.

１・酸化のメカニズムと気を付けるべきこと

1. 氧化的机制与需要注意的事项

１・エアー漏れを放置しない

２・溶接の色がいつもと違ったら報告すること

（溶接部の周辺が黒っぽくなっていたり、茶色っぽくなっていたら、酸化の可能性が高いです）

３・TIG溶接で手直しした際に、溶接部がパチパチと跳ねるような現象になった場合、酸化が考えられます。

このような現象があったら、班長に報告してください

【注意事项】

１・切勿放任漏气不管。

２・如果焊接颜色和平时不同，请立即汇报。
（如果焊接部位周围呈黑色或棕色，很有可能是发生了氧化。）

３・如果在进行TIG焊返工时，焊接部位出现啪啪作响的飞溅现象，则可能是发生了氧化。

如果发现此类现象，请向班长汇报。

【Cautions】

１・Do not ignore air leaks.

２・Report if the weld color looks unusual.
(If the area around the weld looks dark or brownish, oxidation is highly likely.)

３・If the welded part pops or crackles ("spatters") when reworking with TIG welding, oxidation may be occurring.

If you notice any of these phenomena, please report it to your team leader (foreman).
